POLICY PRIVACY OFFICER
Our company’s Privacy Officer is Corwyn Amendt, who is responsible for administering our privacy policy, and ensuring our compliance with federal privacy legislation (PIPEDA) and is the person who will answer your questions and deal with complaints you may have about our handling of your personal information, and he will deal with your complaints in accordance with PIPEDA. PURPOSE
The purpose for which our office will collect, use and disclose your personal information is for any purpose related to your dental benefits and the provision of dental services, under the terms of your collective agreement, your terms of employment, or your membership agreement with WAHM. We will disclose it in accordance with any applicable federal or provincial legislation or with your consent.
CONSENT
After January 1, 2004, for new enrollees, we will obtain your consent one time for the collection, use, and disclosure of all your personal information, in the course of or relating to our work in or providing or making available dental services to you. You may withdraw your consent in writing, given to us at any time and if you wish to do so, we will explain implications of your doing so. If we intend to use that personal information in the future for a different purpose, we will first obtain your consent for doing so. For existing enrollees on January 1, 2004, we will collect, use and disclose, your personal information on hand and to be collected based on the implied similar consent for doing so arising out of our past relations with you. This consent may be withdrawn in the way previously listed for new enrollees.
RETENTION / ACCESS
We retain personal information for as long as the member or his/her dependents remain entitled to dental benefits under your collective agreement or terms of employment, and thereafter for as long as ten years for legal purposes. Each member has a right to have access to his or her own personal information recorded in our records, and we will make those available during normal business hours, upon an appointment being made by the member for that purpose. We will respect your right to challenge or correct the accuracy or completeness of that information, in accordance with the applicable laws.
OPENNESS AND SAFEGUARDS
This statement of our privacy policy is open to anyone and will be made available to anyone, who requests a copy, but your individual personal information is available only to you or to those who are entitled to that information under the provisions of any federal or provincial law. We take reasonable steps, consistent with our office practices, to keep your personal information safeguarded.
